Live in Control is an interactive 4-week program designed to help people with diabetes gain the knowledge, skills, and confidence they need to take charge of their health.Vivir en Control is the Spanish-language version of the program, designed to meet the cultural and language needs of Spanish-speaking participants and Care Supporters.
These programs use a novel Care Supporter approach to diabetes self-management education by intentionally involving a family member or friend who attends classes alongside the participant. This team approach strengthens social support, enhances communication, and encourages sustainable lifestyle changes.
Together, participants and Care Supporters explore practical ways to:
Eat healthier and stay active
Monitor blood glucose levels
Manage medications
Solve everyday diabetes challenges
Reduce health risks
Set and achieve personal health goals
Organizations receive everything needed to successfully offer the program, including facilitator guides, presentation materials, participant resources, and evaluation tools.
